Aluminum phosphide (AlP), commonly referred to as a “rice pill,” is frequently employed as a fumigant for grain storage and is often involved in self-poisoning cases. The clinical presentation of AlP toxicity is varied and nonspecific, including symptoms such as fatigue, vomiting, fever, palpitations, vasodilatory shock, cardiac dysfunction, pulmonary congestion, dyspnea, and potentially death. This case report highlights a rare instance of survival following AlP ingestion. A 13-year-old boy presented with metabolic acidosis and hypotension, subsequently developing atrial fibrillation and flutter, junctional tachycardia and toxic myocarditis.
